A Patient’s Guide to Selective Laser Trabeculoplasty for Glaucoma

A Patient’s Guide to Selective Laser Trabeculoplasty for Glaucoma Image

If you have glaucoma and your doctor has recommended laser treatment, you may be wondering what the procedure involves and what to expect. The idea of a laser in your eye can feel intimidating, but Selective Laser Trabeculoplasty (SLT) is a safe, minimally invasive procedure that lowers eye pressure and helps protect vision. Unlike medications that require daily use, SLT provides a long-term option to manage intraocular pressure effectively. At Eastside Eye Physicians, all of our doctors perform SLT as part of comprehensive glaucoma care in Shelby Township and St. Clair Shores, Michigan.

Why Controlling Eye Pressure Is Critical

Glaucoma damages the optic nerve when pressure inside the eye, or intraocular pressure, becomes too high. This damage is permanent, making early detection and consistent management essential to preserving vision. Many patients begin treatment with eye drops, which are effective but require strict adherence and may cause side effects such as irritation, dryness, or systemic symptoms. SLT offers a complementary or alternative approach that can reduce dependence on medications while maintaining safe pressure levels.

What Is Selective Laser Trabeculoplasty?

SLT is an in-office laser procedure primarily used for open-angle glaucoma and ocular hypertension. In glaucoma, the eye’s drainage system, called the trabecular meshwork, becomes less efficient, allowing fluid to build up and pressure to rise. SLT delivers short, low-energy laser pulses to specific cells in the trabecular meshwork, stimulating a natural response that improves fluid outflow. The surrounding tissue remains intact, and no incisions, stitches, or implants are required, making the procedure minimally invasive.

The SLT Procedure

SLT is quick and convenient, typically taking less than 10 minutes per eye. Numbing drops are applied to ensure comfort, and a specialized contact lens is placed on the eye to focus the laser accurately. While you remain awake and seated at a laser microscope, brief pulses target the trabecular meshwork. Most patients experience little to no discomfort, and the eye pressure may be checked before leaving the office, allowing you to return to normal activities shortly afterward.

Recovery and Follow-Up

Recovery from SLT is usually mild and uncomplicated. Mild redness, irritation, or blurry vision may occur for a few hours to a couple of days, and your doctor may prescribe anti inflammatory drops for short-term comfort. Eye pressure reduction typically occurs gradually over several weeks as the drainage system responds to the laser. Follow-up appointments are important to confirm that your eye pressure is at a safe level and to monitor long-term effectiveness.

Effectiveness of SLT

SLT has a strong safety and success profile, with many patients achieving a 20 to 30 percent reduction in intraocular pressure. Some can reduce the number of glaucoma drops they use, while others benefit from SLT in combination with medication for better control. Its effects can last several years, and the procedure can often be safely repeated if pressure rises again. While SLT lowers pressure, it does not cure glaucoma, so ongoing monitoring remains critical.

Is SLT Right for You?

SLT is ideal for patients with open-angle glaucoma or ocular hypertension and may be particularly beneficial if you:

  • Struggle with consistent use of daily eye drops
  • Experience side effects from glaucoma medications
  • Need additional pressure control beyond drops
  • Prefer an in-office procedure without incisions or implants

A comprehensive eye exam, including pressure measurement, optic nerve evaluation, and visual field testing, is necessary to determine whether SLT is appropriate. Your doctor will help develop a personalized plan to protect your vision.
